Summary

The goal of the presentation is to discuss the meaning and purpose of hospice care, emphasizing its benefits and ways healthcare workers can enhance the care and support to a patient at the end of life. To discuss services provided by hospice. To identify patients that are hospice eligible by discussing general criteria and predicting prognosis.  To discuss specific hospice diagnoses and why they meet criteria for the benefit.  To outline the Hospice Medicare Benefit in regards to what services are covered by Medicare/Medicaid.  To discuss the recertification process that allows patients to stay on service. To describe the 4 levels of care in hospice.  To review a few cases to explain "why hospice , why now?'
