Summary

The SNF QRP required discharge function score measure is the GG replacement measure for the section G based short stay improvement in function. Since January 2024, CMS has been calculating the new discharge function score on IQIES reports. The new measure began to affect 5-star ratings in January ’25 and will affect the Value Based Purchasing incentive multiplier in October 2026. This presentation will help Providers to become familiar with how this quality measure is calculated, how it will impact each SNF quality program, and how to use the results for quality improvement.

The discharge function score is unique as a cross setting functional outcome measure. The complicated nature of the technical specifications make it confusing to providers. As a GG equivalent to the prior short stay improvement in function measure this presentation may, for the first time, clear the air and help providers use the measure data to improve the quality of their short term patient's experience.
