To prevent, detect, and mitigate harm related to medications, Long Term Post-Acute Care providers must have a robust medication review program. How does your organization perform medication reviews? What education is provided to staff administering medications? What processes are in place related to review of medications upon admission? The answers to these questions may prevent harm to residents in your facility. In this session we will review strategies to prevent, detect and mitigate harm related to medications. Ensuring your medication review and education practices are up to date may just save a life!
- Correctly define polypharmacy
- Identify at least one resource to identify potentially inappropriate medications for older adults
- Identify at least one best practice related to medication review upon admission
