Summary

There’s no doubt that clinicians and care teams have been pushed beyond their limits with COVID-19 leaving a path of destruction in its wake. With a vaccine finally here, what comes next? Support for SNFs from government, hospitals, health systems, ACOs, and payors – in addition to the families and loved ones served, has never been more essential for survival. This session will explore various tips and techniques SNFs can begin to implement that use live data already recorded in resident EHRs to strengthen their relationships with partners in the continuum of care. Attendees will learn how to leverage analytics to gain referrals by reducing avoidable hospital readmissions, risk stratify and clinically prioritize patients appropriately, and advance overall care coordination for both clinical and financial gain. Strategies to detect the early warning signs of infection will also be discussed, putting the minds of your residents and their families at ease. Finally, we will also address how analytics can help ensure you are getting paid appropriately for all the care you are providing.
