Summary

To meet the new mandates under the Patient-Driven Payment Model (PDPM), SNFs are being incentivized to develop innovative strategies with their partner hospitals, while finding ways to increase revenue. On the flip side, as value-based care programs gain dominance, the need for acute and post-acute care (PAC) collaboration is greater than ever for your hospital and health system partners.
With a shared goal of improving patient care and reducing costs, a true collaborative effort can be achieved between acute and post-acute care. And, the key to effective care coordination can be found in the data that lives within the PAC EHR.
This session will explore the components of an effective care coordination effort and will reveal how SNFs can easily leverage the data already contained within the EHR to improve clinical outcomes, risk stratify and clinically prioritize patients, reduce hospital readmissions, and stand front-and-center in the continuum of care.
